16:00: An 81 year-old woman was admitted to the emergency department with nausea, vomiting and hypotension. She had a history of hypertension, Type II Diabetes, renal insufficiency and dementia. Home medications: aspirin, furosemide, tolterodine tartrate, memantine, Valsartan, metformin, Lovastatin and niacin. On admission she presented with low grade fever 37.9, a blood pressure (BP) of 60/30 and heart rate (HR) of 130. Lab results on admission included: WBC 16.7k cells/ml, Hgb 9.2 gm/dl, creatinine 1.6 mg/dl, albumin 2.7 mg/dL, sodium 135 mEq/L and potassium 4.7 mEq/L. Abdominal CT was within normal limits (WNL).

State-of-the-art monitoring in treatment of dengue shock syndrome: a case series

BACKGROUND Early recognition and treatment of circulatory volume loss is essential in the clinical management of dengue viral infection. We hypothesized that a novel computational algorithm, originally developed for noninvasive monitoring of blood loss in combat casualties, could: (1) indicate the central volume status of children with dengue during the early stages of "shock"; and (2) track fl...

The effect of selenase on the disease process of patients with septic shock admitted to the intensive care unit: A clinical trial in Tehran

Purpose: Despite discovering new antibiotics, mortality due to septic shock has remained high.   This research has examined the effect of selenase in patients with septic shock admitted to the   intensive care unit (ICU) of a hospital in Tehran.   Materials and Methods: This double-blind clinical trial was done on 80 participants (40 case   and 40 control) who were admitted to the ICU with se...

Advanced Hemodynamic Management in Patients with Septic Shock

In patients with sepsis and septic shock, the hemodynamic management in both early and later phases of these "organ dysfunction syndromes" is a key therapeutic component. It needs, however, to be differentiated between "early goal-directed therapy" (EGDT) as proposed for the first 6 hours of emergency department treatment by Rivers et al. in 2001 and "hemodynamic management" using advanced hemo...

Restrictive versus Liberal Fluid Resuscitation in Children with Dengue Shock Syndrome: the differences in Clinical Outcomes and Hemodynamic Parameters

Background: Fluid resuscitation is the mainstay of treatment to counteract massive plasma leakage in dengue shock syndrome. We aimed to determine the differences in clinical outcomes and hemodynamic parameters of children with dengue shock syndrome post restrictive and liberal fluid resuscitation.  Materials and Methods: A retrospective observational study of pediatric patients who were betwee...

Challenges associated with central venous catheter placement and central venous oxygen saturation monitoring.

Patients with septic shock and respiratory failure often require hemodynamic monitoring in order to guide bedside management. This is particularly true of patients with dialysis-dependent chronic kidney disease, who may be less tolerant of large volume resuscitation than individuals with normal renal function. With the declining use of pulmonary artery catheters and the introduction of early go...
